There is a natural desire to live as much of our lives as possible in our own homes and communities. In fact, 90 percent of seniors living in the U.S. want to age with independence at home, and that is why we are here.

SWIRCA's compassionate and highly trained, professional staff provide specialized, person-centered care and assistance to older adults and those living with disabilities who wish to age in place with better health outcomes and at a lower cost than institutional care. Indiana Pathways for Aging

Launched in July 2024, Indiana Pathways is a managed long-term services and supports initiative for Medicaid enrollees (or those dually eligible for Medicare and Medicaid). Eligibility requirements for the program include:

  • 60 years of age and older
  • Eligible for a full-coverage aged, blind or disabled category (with or without Medicare)
  • Can receiving long-term support services including:
  • Resides in a nursing or long-term care facility
  • Approved for an Aged and Disabled Waiver
  • Can be on the Behavioral and Primary Health Coordination program

INDIANA PATHWAYS OR HEALTH AND WELLNESS WAIVER WAITLIST

To confirm that you have been added to the Pathways or Health and Wellness waiting list, you may visit the Online Dashboard. Please note it will take at least five business days for your name to be added to the waiting list. Unfortunately, your local Area Agency on Aging will not be able to tell you about your place on the waiting list, nor will FSSA, but can provide information regarding your level of care date or priority category status.

TRAMATIC BRAIN INJURY WAIVER WAITLIST

The Family and Social Services Administration's Division of Disability, Aging and Rehabilitative Services (DDARS) has implemented a waiting list due to the waiver reaching maximum capacity. As approved by the federal government, 200 total slots are available during the timeframe of January 1, 2025 through December 31, 2025. Please note, members currently enrolled in and receiving waiver services will not have their services changed or interrupted by the implementation of the waiting list. For questions regarding the waiting list process and placement, please contact BDS.Help@fssa.in.gov.
