Person-Centered Advocacy

We understand the challenges that come with researching, understanding, and coordinating long-term care support needs. Using a team based, person-centered approach, our Care Managers conduct comprehensive, in-depth assessments of all individuals who seek long-term care coordination and in-home services. Care Managers support funding recipients by giving them choice and control as they examine options available to them to meet their wellness goals.

Once services are in place, Care Managers then provide continuous support, monitoring client progress against person-centered goals and adjusting care plans as functional needs change.

Thanks to the incredible efforts of our Care Management Department, we’ve already completed more than 600 service plans in 2025—and we’re doing so with an astounding success rate of over 99%! This achievement not only showcases the dedication and skill of our team but also ensures that the people we serve receive smooth, uninterrupted care.

On top of that, it helps guarantee seamless claims and service delivery for the waiver and non-waiver providers we proudly partner with. Our care managers are truly raising the bar and setting the tone for excellence in 2025!

Miranda, In-Home Waiver Supervisor

I'm very passionate about the goal of person-centered care and what it means for our clients overall health and wellbeing. Each individual we serve is unique, and we as Care Managers, do all that we can to ensure our clients receive the very best care.

Care Managers handle approximately 60-65 cases at one time and assist clients ranging in ages from infancy to 100 years plus. We work with each client to build positive rapport and trust. We discuss their person-centered preferences for care, assess the current home environment, monitor their overall health, and assist with creating and realizing SMART goals (Specific, measurable, attainable, realistic, timely)

Although at times in can be tough, the job we do, above all else is extremely rewarding. We have helped many escape homelessness, danger, and hunger. We have sat with families and cried with them through times of joy and sorrow. We have advocated for those who needed a stronger voice!

We are on the heels of an exciting and somewhat overwhelming time, as the largest generation of seniors we've seen in our lifetime will begin to need our services. I am excited about this challenge, and look forward to facing it together.
